Modem/laptop set-up with Mandrake 10

Hello;
New to Linux; Installation went well however not able to complete settings for Xircom 56t.modem(dial-up) When i get to the step "Select your provider" there are countrys that are listed but not the "US" (There may be a good reason; This current administation does not consider other countrys. I'll leave this alone)

I have checked the Mandrake web page to determine compatablity with modem and indicated that other users determine yes.

Not sure which com port. How can i determine?
Clean installation after scandik-fdisk.
Computer is: Gateway solo laptop-pen11-400mz-4.5 gig
Thank you for your attention.....tom reid
