installation of rhel 5 from usb displays "can not find iso file" for installation

i have created rhel 5.4 iso by using an iso creater in windows 7 from rhel 5.4 OS DVD.
then i followed the procedure to create USB installer

created a partition on USB using fdisk command with vfat file syatem.
# mkfs.vfat .dev/sdb1 and mounted USB under /mnt/usb
# cp rhel-5.4.iso /mnt/usb and other files.
# syslinux /dev/sdb1
# mv isolinux.cfg syslinux.cfg
# grub also installed

But on installation from USB it displays " disk driver image find on multiple devices or multiple partitions" on selecting any device or partition it displays " installation aborted and safe to reboot"

Please give procedure to create USB for installation as DVD and installation method as like as DVD

First off RHEL 5.4 is VERY old .The current in the old 5 series is RHEL 5.8 ( 5.9 is in testing)
And the current release is RHEL 6.3

Also RHEL is not really a free operating system.
You MUST buy a "support contract / License"
for the desktop license it is from $49 to $299 PER YEAR
https://www.redhat.com/apps/store/desktop/
basically if you do not buy the license you do not install software

or

Install one of the FREE rebuilds
-- for the rather old 5
CentOS5.8 -- might NOT install on new hardware ( to old )
ScientificLinux 5.8 -- might NOT install on new hardware ( to old )

--- the current --
CentOS 6.3
ScientificLinux 6.3

for installing by a usb thumb
