Importing Address CSV file Into Thunderbird

I exported address information from Outlook as a CSV file and imported it into Thunderbird. However, the field information is not in the proper places after the import.

How can the CSV file be edited in order to put the correct information into the proper Thunderbird fields when importing?

This page has some good info on the fields thunderbird expects:

To edit the actual csv, you can use something like mousepad or kate (depending on your setup), or you can download open office and use calc
