Please check your cache.log and access.log Squid log files, by default located in the '/var/log/squid3' directory, as they generally provide insight as to what is causing the erroneous behavior.

Like :

tail -f /var/log/squid/access.log

That would show you what is comming back. Do you have a forward loop or anything?

tail -f /var/log/squid/cache.log
