Iptables - Redirecting Outgoing Traffic

How can I use iptables to redirect all outgoing connections to XXX.XXX.XXX.XXX to YYY.YYY.YYY.YYY?

I've tried iptables -t nat -A POSTROUTING -d XXX.XXX.XXX.XXX -j NETMAP --to YYY.YYY.YYY.YYY but it doesn't seem to work.

The target is called DNAT (Destination NAT) not NETMAP.
