A way to forward an IP/Port to another system's IP/Port

Can I forward traffic coming to an IP/Port on one system to an IP/Port on another system? Basically just forward it over - I want to move a game server from one system to another system but I want people to be able to connect with the old IP address.

I don't want the traffic routed through the first server at all, I just want it to bounce them to the new server.

Can you just swap the machines IP addresses ? That would be the quick fix....

Unfortunately no - the systems are hosted in a datacenter and they do not allow that.

I guess my question should be revised to ... Does using IPTables still put the traffic through the first IP address / system? If so, is there a way to re-route all traffic so that it is not going through the initial IP address at all?
