You'll need to install 'imwheel' - have a google around.
The following will get all the buttons working for KDE, but you can put the shell script described in Gnome's auto start folder or whatever.

Install imwheel
Modify XF86Config Mouse part to look like:
Option "Protocol" "ExplorerPS/2"
Option "Buttons" "7"
Option "ZAxisMapping" "6 7"
Leave the rest unchanged.
Stick the following shell script into .kde/Autostart:
#!/bin/bash
xmodmap -e "pointer = 1 2 3 6 7 4 5"
imwheel -k -b "67"
create $HOME/.imwheelrc with the following contents:
".*"
None, Up, Alt_L|Left
None, Down, Alt_L|Right
restart KDE.
