imps2 mouse doesn't work with KVM switch
I have a Belkin KVM switch and I got it all setup and everything and everything worked fine. A few days later when I restarted one of the computers, the keyboard worked fine but then when I typed 'startx' I couldn't move the mouse. I restarted the computer and tried again and again and again with no luck. This happened on the other computer, but the mouse always worked after two or so restarts. I have Slackware 9.1 and running KDE

I can't say this will solve the problem, but I used a Belkin to swtich between Linux and Windows, and the mouse would always successfully go from "A" to "B", but if I then tried to go back to "A", it hung just as you described, regardless of which OS I started with.
Solved the problem by replacing the Belkin with a Trendnet TK-401R, which works flawlessly. -- J.W.

Try a different mouse.... Yeah, same KVM and same system, but different mice types (both the XFree config type and the actual hardware) respond differently to being switched.

Check if you are using a PS/2 -> USB converter
All of my KVM problems went away when I noticed that the actual mouse I was using was connected to the
KVM through one of those PS/2 (round plug) to USB (rectangle plug) converters.
I found another mouse that didn't need the converter, but still has the wheel and everything works great.
For me the IMPS/2 didn't matter and getting rid of the scroll wheel in XF86Config didn't help but getting that converter out of the way made everything work like magic.
