How to create link to modem?

How to create link to modem that doesn't disappear every time I reboot?
I have installed modems (ltmodem and ACM) and they work fine but when I restart computer links /dev/modem for ltmodem and /dev/ttyUSB0 for ACM are gone.I think udev rewrites on every restart /dev directory.
I add modules ltserial and cdc-acm in /etc/modprobe.preload and they are loaded now but where to add something about links?

Edit one of udev scripts that detects the manufacture ID and product ID that will create a device node called modem or what ever name you want when the modems are connected. Finding the right script in /etc/udev.d and learning the udev syntax will take some time. Hotplug can also do this but if you have udev, it is better to use it (udev).
