Debian stable/IIyama Vision Master Pro454 - refresh rate too low


I'm using the latest Debian stable. I have:
HM903DT IIyama Vision Master Pro454 monitor
nVidia Quadro FX 1300 video card

I have searched around on google and in the forums for answers to my problem. I've seen a couple similar problems, but they were either resolved via things that haven't worked for me or no one posted an answer.

I'm using the latest nVidia drivers. When I start X, my monitor refresh rate is limited to 85 Hz regardless of the resolution. My monitor, according to the specs, can do 800x600 at 200Hz and 1024x768 at 162Hz. I have tried adding various mode lines to my XF86Config-4, but these invariably result in problems around the edges of the image. Here are the relevant sections of my XF86Config-4:

None of the vertical refresh rates breaks 87 Hz.

I am told that I need to be able to get at least 120 Hz to use the stereo capabilities of the video card correctly. I am looking for some suggestions on things I might do to enable higher refresh rates or pointers to areas with more information about EDID, monitor refresh rates, etc.
