3ware raid card problem. Disk disappears when the rebuild of the array starts


Hello everyone,
I'm in the dark here.
I seem to have a bit of a problem with a RAID1 I have configured on the 3ware 9650SE-4LPML.

from it's 4 ports I have created 2x RAID1 arrays. (OSarray and DATAarray)


I seem to be losing the 1 of the 2 OSarray disks. The same one always.

After a restart, on the 3ware bios screen I will see the disk present but 'not in use' in the array and the array will be degraded.
once I start the rebuild process, the disk will disappear ('not present') and dmesg will output:

restarting the pc will probably show the disk again and after I start the rebuild the disk will disappear again.
At some point in time I managed to complete the rebuild and would have the system up and running for 2 months or so (it's not in production yet I am doing tests and including functionality). lately I had to restart it again and we got in that degraded phase.

My guess is its either the disk's fault, or the PSU (850W, but I have 8 disks attached to it). but I wanted to ask around if there is something else I could check, before I go out and buy a new PSU and disk.

Did you check all cables?

yes, of course.
I the end I replaced the hard drive.. even though I wasn't sure It was the cheapest and fastest attempt for a solution.
it turns out that was the problem.
