What's the term of?

Ok guys, here it goes. I want to be able to create a webpage or a series of WebPages that have input boxes for like
name, first and last, Middle Initial
ssn#
Phone number
Address
Position
Office location and stuff like that.
I want a submit button so that when you hit submit it will then populate a database with that info.

The forms I want to create will be a bit more than just that.

This is going to be a page where the hiring manager can fill this out and when he submits it, there should be an email sent out to the sysadmin for account creation
telecom for phone at desk
Facilities to get the office ready
IT to have a computer and monitor, keyboard, mouse and what ever build is needed.

I'm so freaking tired of having to get a form off our website and fill it in, print it, fax it
and weeks later you have your new freaking employee sitting at an empty desk, no user accounts, no puter, no phone, nothing.... because some AH either dropped the fax into the garbage or whatever.

I know the LAMP part... but what is it called that I want to do..?

Blonde Moment here.
I think in the Microsoft world it's called ASP,
What is it in Linux????

Thanks

It's the bestest web scripting language on the planet, and it's called PHP. http://www.php.net/. Good luck!
