Quick Mail Envelope Question


So in my mail envelope (I believe this info is contained within the envelope) I have this as my first received line:

Received: from unknown (HELO docnotes.catholic-doc.org) (67.33.15.34) by
mail-hub.123nc.com with SMTP; 17 Jun 2004 21:50:04 -0000

My question is, what is the host docnotes.catholic-doc.org role in this piece of information?

I understand that you are supposed to say "helo" to an smtp server when you make a connection. Is it safe to assume that this hostname can be easily faked and should not be considered as real? And notice that it says "unknown" prior to the helo info. What *should* be in place of the keyword unknown?
