Nvidia driver not working in Mandrake 9.1

Hello all.

Newbie here.

I apologize for posting yet another nvidia driver thread, but I think I've read through every other one on here, and I can't seem to find an answer.

My system:

Athlon XP 2500
Epox 8RDA motherboard
Geforce 4 Ti 4200
Mandrake 9.1

I have read and followed the readme instructions from nvidia. I followed the tutorial posted on here (http://www.linuxquestions.org/questions/answers/30). All I ever end up with when I type "startx" is either a completely black screen, or a black screen with several horizontal blue lines across the bottom.

Not necessarily in this order I have:

...downloaded NVIDIA-Linux-x86-1.0-4496-pkg2.run, and it claimed to have installed successfully

...changed "nv" to "nvidia" in XF86Config-4
...made sure I have "Load 'glx'"
...checked to make sure I don't have "Load 'dri'" or "Load 'GLcore'"

...altered /etc/inittab to start in run level 3

...checked my bios to make sure assign irq to vga is enabled

...tried typing "/sbin/modprobe nvidia" before trying startx

...checked to make sure I have accurate refresh rates for my monitor and tried starting x at 800x600

Always I get the black screen/black screen with blue lines across bottom. I always have to do a hard reboot to get out of this.

Is there something I'm missing? Has anyone else experienced a similar problem?

Any help is appreciated.

Thanks.
