What does this indicates?

Hardware issues with your HDD, perhaps? Can you access it at all, or does it just throw those errors and fail miserably?

If you can access it, things to check for would include:

1) Funky noises when it tries to spin up
2) Slower than usual access times
3) Does fsck have anything useful to say? 'fsck /dev/hda3'

Everything seems fine physically and we can still access everything on the disk, is there any relation to the kernel or DMA?

like es0teric said looks like a physical disk problem, maybe some bad blocks. I would recommend you running fsck to try to find more info about this errors and if the disk content is important to backup it the soon as possible.

take a look at http://linux.die.net/man/8/badblocks

By the other side, have you done any change to you LBA settings in the BIOS trying to correct any bad bios disk size guest?(Normaly we did that in the past with old disk that doesnt have a full LBA compilant)
