How can I customize my terminal's title bar?

I'd like to have my terminal's title bar show the IP address of the machine I've connected to using ssh. The machines I connect to have multiple NICs though. So if it only showed the IP address that I connected on, that would be perfect. Can anyone tell me how I can do this?

And what terminal are you using?
The method to perform this would certainly rely on this as the single most important factor. For example, xterm, putty on windows, text console on linux machine, etc.

edit: Echoing $TERM gives me xterm. But if I hit alt+f2 and type xterm, I get a completely different looking terminal even though both give me xterm when I echo $TERM.

It is gnome terminal. I've been searching around and I think I have to set the custom command option under the preferences UI. But before I make that I need to know what the normal shell command is. Any idea where I can find that?

The GNU Bash Reference may be useful.
