I had a lot to work. That is why I am answering late. lvm2 has been yet installed and lvdisplay does not show any volumes:
Code:
$ sudo lvdisplay 
  No volume groups found

$ sudo lvdisplay /dev/dm-0
  Volume group "dm-0" not found
  Skipping volume group dm-0

$ sudo lvdisplay /dev/dm-1
  Volume group "dm-1" not found
  Skipping volume group dm-1

$ sudo lvdisplay /dev/dm-2
  Volume group "dm-2" not found
  Skipping volume group dm-2

$ sudo lvdisplay /dev/dm-3
  Volume group "dm-3" not found
  Skipping volume group dm-3

$ sudo lvdisplay /dev/dm-4
  Volume group "dm-4" not found
  Skipping volume group dm-4
I don't know how to interpret the dmsetup output:
Code:
$ sudo dmsetup ls
pdc_efghjije	(252, 1)
pdc_gcdjbfhd	(252, 3)
pdc_dfdcfcaf	(252, 2)
pdc_gcdjbfhd1	(252, 4)
pdc_ehfiaedh	(252, 0)
This problem is so weird.

Lets clear all our confusion and work on narrowing down the issue:

1. Do cat /proc/mdstat (Will show if any RAID is configured or not)
2. Do a vgscan (Will show us if any volume group other than the default one exists or not. If exist then we can check under /dev/volumegroup if there is any logical volume)

Doing above will help us to understand if any advanced partitioning involved or not. If above results are negative then we can concentrate on basic partitions and work in that direction.

I have now put the controller cards physically into other slots. This did not change anything.

I had the time to work some more hours on my problem. By chosing a suitable master/slave jumpering I could access another disk. I am still trying different combinations to bring back the remaining two.
