Doom 3 video setting questions.

I have three questions about the video settings.
1. whenever I start the game all of my advanced video setting reset and I have to turn them off(the only thing I use is bump map)
2. In multiplayer the advanced video settings are all turned on and I can't play(too laggy) is this normal? does the server control that? and does anyone know of any servers without all these settings?
Anyone know of a inexpensive card that runs doom 3 faster than a geforce fx 5200? The lighting kills this card, and I don't want to dish out $400-$500 on a video card.

For a new video card, try a GeForce 6600 GT. $200 and as powerful as the 6800 Ultra in some cases.
