mutt and clamav

I fetchmail my @gmail.com mails and read via mutt and send via msmtprc. I would like to add clamav so all outgoing mails get checked by clam and add an X-header. Would anyone know how to do this? (I know google checks for virus but I want to do this for learning purposes)

Google search didn't give much on this subject. Thanks for any help.
