You can very easily make such an image using GIMP, Photoshop, etc.

Or, it could be that the Flash player stopped responding and failed to redraw the portion of the screen where an error message once was.

EDIT: is that a screenshot of your desktop (since it looks like there is an LQ page in one of the tabs)?

Oh I assure you, it's real...I just took a screenshot of only the FF window.
