I am afraid I do not get the point of posting a link that is almost two years old---especially when you do not even explain how it relates to your thread title.
The Upload box in a standard HTML form. I'll admit, what with all Ajax and whatnot they're becoming less common. But try using the Attach Files functionality on this very forum and you'll see what we mean.
I've attached a file to a post more than 7 times in the last few days, and I didn't find any problems. I'll pay more attention next time and look for problems.
I'm not sure if it's to do with the file chooser. Rather, the problem is you can't just type or copy-and-paste the file's location into the text box. You could in fx2. Very useful in some situations; for example if it's multiple uploads with very similar filenames.

I still don't get what window you are talking about. I posted a screenshot of what I see when I upload attachments:
The LQ Attachments window. Do you see the greyed-out box to the left of the Browse... button? In firefox 2 that was a real textbox, that allowed you to type in a file location, for example "/home/user/docs/pictures/pizza.jpg", without ever having to open the file chooser at all. In firefox 3 the moment you click it the chooser pops up.
