If you've seen the news you'll know the three letter agencies have near-global jurisdiction, so it doesn't matter too much where the servers are. I mean what country would you recommend ?

Switzerland or Sweden, because I understand their privacy laws are more stringent.

This sounds interesting, can you explain more about doing it and the best place to go? Any cons to doing this?

Purchase a domain name using a registrar like Gandi, connect it with freedns, then find a reputable/privacy-oriented mail- or VPS-provider (preferably located in Switzerland or Sweden, as mentioned above) and configure it as necessary depending on the provider you choose.

Laws? The UK has laws against this kind of thing but it doesn't stop our government selling us to the highest bidder.

I'm sure Gmail will do the same soon, but I have stopped using it a long time ago.
In fact g-mail still has this feature but optional.

Outside of corporate accounts where people purporting to need real security get a number on a special device, I don't see any real need for this (while signing into your mail). I think that for google, ms and all the others, it is a way to assert that they have take all possible steps to avoid unauthorised logins.

OK

OK

Mostly phones/tablets (Google's Android, ...) and game consoles (Microsoft's Xbox).

Those accounts are used for your tablets, phones and game consoles as well... (not saying that's a good thing).
Would it be fair to dump everyone who doesn't own a PC/MAC/Whatever.

That and pink ponies.
