I had this same problem recently, and when I finally got fed up with it, nothing I found anywhere would work, until I learned Gnome<-->GTK. FYI, I'm on openSUSE 13.1. I think someone else might still be pulling their hair out over gnome icons missing, so it's time to share my notes:
In the System Settings (KDE menu item is called Configure Desktop) under Common Appearance and Behavior; Application Appearance, use any of the GNOME icon theme for GTK applications. If not, icons will be missing from Gnome applications. Fallback theme doesn't seem to matter.
