Asterisk?

I know it's not exactly "just a desktop app," but I consider it the "be all, end all" of VOIP applications.

SFLphone is great: has a lot of options, a cute interface, and can even record all calls automatically!

skype

skype is useful. tried other foss options,,, ekiga, linphone but not happy.
