The 3D Prophet 4000XT card, is a card from hercules, it's a standard PCI card (There was also an AGP version I think) and it was made around 2001, and the only official kernel module for the card is for the 2.4 kernel, but it's completely VGA/VESA compatible, so it doesn't really matter.
There is a framebuffer module for it though, "kyrofb".
